Sprout Health Texas understands the stress and anxiety potential patients are feeling during the initial stages of the recovery process.
We have created an effortless pre-admission and intake process. Our intake coordinators are skillfully trained to communicate effectively with both potential patients and their referring doctors.
We accept referrals from multiple healthcare organizations and, with our numerous resources that are designed to aid both clients and referrers, we help facilitate a seamless transition to our facility. Our resources will be available during the admission, treatment and discharge phases in order to keep open communications between our facility, patients and the referring health care provider.
We pride ourselves on our fast and straightforward communication in order to properly receive the necessary documentation and medical information regarding prospective patients.
Our Facilities and Admission
The majority of common patient admissions systems are slowed due to regulatory processes and we strive to relieve those challenges. Sprout Health Texas provides all referring individuals, professionals and organizations with accurate information and guidance pertaining to our treatment centers and programs.
Once a client chooses to receive treatment from us, we will contact the referring health care professional to gather as much necessary information pertaining to the client in order to develop a custom treatment plan catered specifically to their particular needs. We take into consideration health and addiction history as well as any underlying conditions that may have been diagnosed during our assessment stage.
Because of the Health Insurance Portability and Accountability Act (HIPPA), we adhere to the required ethical and legal guidelines of confidentiality. Any information we garner during our assessment process will remain private unless otherwise stated by our patients. Contributing medical professionals may want updated information on the client’s progress; however, some information cannot be disclosed; therefore, we will request written consent from the client. We respect referrals from the medical professionals; however, we also respect our client’s right to privacy.
The Importance of a Discharge Plan
Regardless of consent or not, all authorized parties are notified of our patient’s discharge plans. The goal of discharge planning is to prepare our clients for their release from our facility as well as notify health professionals of their impending release. We want to prepare everyone for what is to come. Our referral coordinator will communicate the arrangements to the referring health care provider.
